1 – REGISTRATION

We will need your insurance information in order to verify your physical therapy benefits. It is preferable that we receive it in advance when we contact you by phone. In either case, you will need to bring your insurance card with you on your first visit.

2 – VERIFICATION

Using the health benefit information you provide us, we’ll contact your insurance company directly to verify your specific physical therapy benefit. In doing so, we will also confirm whether any of your allowance has been used year-to-date and, if so, determine your balance available for further treatment.

3 – SUMMARY

We’ll provide you with a written summary of your benefit verification when you begin your therapy program.
