We know you are already in pain. That is why we have developed a system to eliminate potential roadblocks so we can get you in and out of the clinic quickly and easily.

  • We speak on your behalf with insurance companies and file all necessary forms you have filled out.

  • You schedule a time to meet that works best for you.

WHAT TO EXPECT

  • CHECK-IN

Welcome! You will be introduced to our staff and given a tour of our facility, equipment and changing room. We will review your insurance forms and contact your insurance provider on your behalf to solve any outstanding issues.

  • EVALUATION

Your evaluation forms the basis of your treatment. We’ll ask you questions about the history of your problem, how it bothers you, and how it limits you. Then we’ll perform tests specific to your complaints/condition. Finally, the findings will be explained to you and your questions will be answered.

  • TREATMENT

Our initial treatment will be based on your examination and you’ll begin to learn your home exercise regimen right away. During each clinic visit, your treatment program will be scrutinized and updated according to changes in your status. As you meet short-term clinical goals, both your in-clinic and home exercise programs will be advanced.

  • LENGTH OF FIRST VISIT

Although you should plan to invest 60-90 minutes for your initial session, we would be happy to shorten your visit to satisfy your schedule.

  • SCHEDULE

Before you leave, we will have in-place a therapy schedule which works best in your busy lifestyle.

WHAT TO BRING

  • PAPER WORK

  1. Download the appropriate Allegiant PT First Visit Form file below.
  2. Print the forms and fill out by hand.
  3. Fax the forms to 603.397.5538 or bring printed forms with you to your first visit. For your personal privacy, please do not email the forms.
  • INSURANCE CARD

Please bring your insurance card with you.

If you have provided us your insurance information in advance over the phone, we’ll make every effort to have verification of your PT benefit allowances for you when you arrive.

  • CLOTHING

Please bring or wear loose-fitting clothing appropriate for exercising — and proper footwear, as well. In addition, for certain types of problems we request:

  • NECK – Tank top
  • SHOULDER – Tank top or loose-fitting T-shirt
  • HIP – Loose or baggy shorts
  • KNEE – Shorts
  • FOOT / ANKLE – Shorts. Also, bring any footwear you wish to have evaluated.
